Apparently, there will be an updated version of Life on a Line available in late 2005. Another good publication is Considerations for Rope Rescue by Ken Laidlaw. The current version is from 2002, but this may be getting updated soon too.

For those interested in attending formal cave rescue training (in the US), the National Cave Rescue Commission now has some documents available on their website at http://www.ncrc.info If you click on "student resources", you can download the NCRC skills testing criteria for knots, rigging, SRT and so forth.
