This former commercial cave is now a vertical trip. It includes ice formations and weird old tour structures.

Giving away the state might make it too obvious, but I will disclose it if no one can guess without it.

Nope, it's the one near Craters of the Moon in Idaho. It was featured in Halliday's Depths of the Earth

Nope, try again. This is a very popular wild cave (it is my favorite cave in Indiana) there is a little bit of everything in it. One last BIG hint: this cave is normally done as a through trip.
