Miscollations in a publication occur as a human error when the sheets are loaded. Usually they affect only a single signature (small number of copies), so it could well be that this error only affects copies going to Indiana.
But it's possible that multiple signatures got screwed up. Hopefully not more than the usual number of extra copies that are printed against such contingencies. Just ask for a replacement if you got a bad one. Better yet, save the Society a couple of bucks and ask Bunnell to send you a pdf copy.
But the formatting on pages 7 and 11 are NOT printing errors. Margin bleeds are often used by Dave B. as a formatting device and to squeeze more or larger images onto a page than would otherwise be possible. Get over that.
